participation requirements

Welcome to LJPC! It is our honor to host your child at one of our weekly programs. HUGS (Pre-K - 1st Grade) meets on Wednesdays from 3:30PM-4:30PM, and XZONE (2nd - 5th Grade) meets on Wednesdays (5PM-7PM).

To verify it is a parent/guardian completing these forms, we require a copy of your ID is submitted. A photo of your ID can be emailed here. Alternatively, you can bring your ID to the church office.
